Too many of our youth are facing challenges that put them at risk. Join 18 Degrees in connecting our youth to opportunities and resources to live their promise and potential. Case Managers in our Pittsfield Community Connection (PCC) program help youth 17-24, including youth involved in/or at risk of gang activity, violence or other criminal behaviors to succeed and avoid negative impact on their futures. Through their relationship with the program participant and an assessment the Case Manager identifies participants’ needs, goals and strengths, tracks progress; coordinates needed services; provides referrals intra-agency and to other community based providers, and provides individualized support and advocacy.
Required: Ability to build trusting relationships while maintaining appropriate boundaries; knowledge and understanding of the systemic, socio-economic, educational, behavioral health and other challenges our youth confront including trauma and knowledge of gang culture; ability to meet individuals where they are; build community connections; and work as a member of a supportive team. Lived experience and multilingual/multicultural fluency are a plus. Prior experience with proven risk youth and case management experience a plus. Competitive pay rate and benefits.
