Service Coordinators drive the everyday work of Family Networks and are responsible for the placement and case management of youth in various programs.
The role:
- Manages caseloads of families, children and youth
- Develops and maintains a strong partnership with the Department of Children and Families (DCF), agencies, programs and families
- Coordinates services to achieve goals and outcomes of services identified in the DCF service plans
- Monitors service implementation; and ensures coordination with providers and DCF
If you are passionate about creating respectful partnerships that help families find their own path then we want to hear from you. Associates Degree required, BA in a social work or a related field preferred. Experience required: family-centered welfare related experience, and/or community-based experience.
