Outreach Case Manager - Pittsfield Community Connection


18 Degrees (formerly Berkshire Children & Families, Inc.) is where new beginnings come to light for children and families – and could be where your new career does the same! 18 Degrees, a dynamic non-profit agency serving Western MA, is looking for an Outreach/Case Manager for our Pittsfield Community Connections GVP – program. • The Outreach/Case Manager reports to the GVP Director – Outreach/Case Manager will identify, engage, and recruit youth to the GVP program. • The Outreach/Case Manager supports youth (program participants) throughout their involvement with the GVP program. • Orients youth to the program, develop a plan for their participation, and pursue the goals they have identified. • Work with youth to develop both soft and specific skills to ensure that they are able to participate in a goal-oriented program. • Connect youth to other resources and services as needed, and will follow-up to ensure connections are established. • Facilitate groups for youth to help them identify and pursue personalized goals for wellness, job readiness and financial success. • Maintain communication with other stakeholders where applicable about the youth’s progress, setbacks, and successes. Qualification Requirements: Education Preferred: A.S. preferred or equivalent experience Experience: 3 years experience working with At Risk and/or Proven Risk youth preferred. Experience working within local community agencies/employers in a human services capacity (with gun violence and/or gang intervention/prevention experience preferred). Lived experience is valued and considered. 3 years' case management experience strongly preferred. Data collection experience, a plus. Strong computer and internet skills, including Microsoft Office. Employment Manager - Gun Violence Prevention Program


The Employment Manager works with youth (program participants) and GVP Case Manager to assess participant job-readiness, aptitudes and interests; consults with the GVP Case Manager on skill-building to create job-readiness plans for program participants. The Employment Manager serves as liaison in the community to meet/engage business owners and prospective employers; orient them to the GVP program and mission; and develop and maintain relationships/arrangements for youth in the program to receive training, subsidized employment and/orjobs. Qualification Requirements: Education Required: B.A. in education, human services, or related field strongly preferred. Minimum Required: High School Diploma or equivalent with at least 2-5 years' experience with community employment training/assessment/placement for youth populations in organizational, institutional and educational settings. Experience: Gun violence prevention and/or gang intervention/prevention experience preferred; Strong computer and internet skills including, Microsoft Office; Strong interpersonal communication skills. Program Director - Gun Violence Prevention Program


The Program Director - GVP is responsible for representing the program as the Community Backbone and for building the Community Coalition of organizations and agencies that are committed to addressing this problem. The Program Director - GVP will develop and implement the Youth Advisory Board and, with their input and oversight, the community strategy. The Program Director – GVP has responsibility for GVP program operations, as well as the program budget, data collection and reporting, and the administrative aspects of the GVP program. Works in direct, partnership with Senior Director and Program Director of Pittsfield Community Connection to oversee and maximize the coordination and collaboration of gang intervention and gun violence prevention programming, efforts, and services. Qualification Requirements: Education: MSW or equivalent, strongly preferred. Minimum Required: Associates Degree or higher in human services or related field. Experience: 2-3 years of program-supervisory experience working within the local community agencies and employers in a human services capacity. Gun violence and/or gang intervention/prevention experience preferred. Strong computer and internet skills, including Microsoft Office. Strong interpersonal communication skills. License(s) Required: LSW, LICSW, preferred. Competitive salary and benefits.
