Family Networks Service Coordinator - Greenfield [30 hours]

Service Coordinators drive the everyday work of Family Networks and are responsible for the placement and case management of youth in various programs. The role manages caseloads of families, children and youth; develops and maintains a strong partnership with the Department of Children and Families (DCF), agencies, programs and families; coordinates services to achieve goals and outcomes of services identified in the DCF service plans; monitors service implementation; and ensures coordination with providers and DCF. If you are passionate about creating respectful partnerships that help families find their own path then we want to hear from you. Clinician for Youth Programs

As an employee of 18 Degrees, you would work in both our Pittsfield and North Adams programs helping youth 17-24, including youth involved in or at risk of gang activity, to succeed and avoid negative impact on their futures in a proven program model that builds and supports positive connections and activities, along with employment & education opportunities. Required: LCSW, LICSW, or LMHC or equivalent; experience (work/internship/volunteer/military) working with youth/young adults; understanding of the systemic, socio-economic, educational & other challenges our youth confront; work as a member of a supportive team from both locations on a regular schedule. Family Services Access Specialist

The Services Utilization Specialist in our Family Networks program supports access to needed services for children and families by facilitating referrals to providers, tracking referrals & placements, maintaining & communicating updated reports, balancing budget support for services, and coordinating closely with 18 Degrees’ Family Networks team as partners to the Department of Children & Families (DCF). Required: Highly proficient in Excel spreadsheets, building reports, data entry & MS Office skills; interpersonal skills, super organized, effective & flexible in a changing, fast-paced environment. Associate’s degree OR substantial experience. Bilingual Spanish/English adds value. Willing to split workweek between our Pittsfield & Greenfield sites. Background check required. Family Support Specialist - Berkshire
